Transfusion Reaction Investigation
Test description
A laboratory clerical check of all documentation relating to the pre-transfusion sample is performed. Pre and post- transfusion venipuncture samples are visually inspected and compared for the presence of hemolysis. Preliminary tests may include the determination of patient's ABO group, Direct Antiglobulin test (DAT) and a screen for the detection of atypical antibodies. Additional testing is performed based on the patient’s clinical symptoms and/or laboratory findings.
Specimen and requisition requirements
Specimen(s):
Minimum of two (2) 4 - 6 ml EDTA (lavender) tubes, unless pediatric patient, mixed thoroughly by gentle agitation.
Label specimen with the required minimum information: patient’s last name, first name, PHN or Unique Lifetime Identifier (ULI) and date of collection.
Complete Requisition (must include):
- Patient’s last name, first name, date of birth and PHN or ULI
- Facility name, complete address, phone and fax number
- Phlebotomist ID information
- Date of collection

Transfusion Reaction Details
- Reaction date, clinical signs and symptoms, pre & post vitals, measures taken
- Blood component blood group, component type, donation number, volume transfused, transfusion start and end dates and times
- Nursing clerical check

Recommended Refrigeration 1-10°C.
