Requests for access to personal information under the custody and control of Canadian Blood Services can be made by submitting a written request. To make a written request, please print either Form A or Form B below, as appropriate:
When completing the form, please be sure to provide:
Sufficient details regarding the nature of your request, for example, blood type confirmation;
Your handwritten signature on the form; and
A legible copy of your Canadian Blood Services Donor ID card (front and back).
If you do not have a Canadian Blood Services Donor ID card, please provide a copy of an alternate piece of signed identification (front and back).
Your request will be processed within 30 days of receipt by the Access to Information Office. If we cannot process your request within 30 business days, the Access to Information Office may extend the time limit for a further period of no more than 30 days. The Office will notify you in writing of an extension with an explanation.
Sending information electronically may result in the unauthorized viewing, use, disclosure or distribution of your personal information during transmission. Should you choose to submit your Request and Consent for Release of Personal Information form electronically and/or choose to have Canadian Blood Services provide your personal information to you or a third party electronically, Canadian Blood Services assumes no responsibility for the security of your personal information during transmission.
