Application Guidelines


The "Request for Cord Blood Products for Research" application form (PDF) consists of four sections, as outlined below. To obtain the “Permission to Collect Form” and the “Information for Cord Blood Donation for Biomedical Research” pamphlet, click here (PDF).
 

Section 1: Research Project Information

Project Title

The fundamental nature of the research study.

Principal Investigator

The researcher who is requesting cord blood products and is responsible for managing the project and for progress reporting to the Cord Blood for Research Program.

Contact person for the project

A delegate for the Principal Investigator (if applicable) to whom project correspondence should be directed.

Organization/Research Institution Information

Where the research project will be conducted.

Co-Investigator(s)

Researchers identified on the research project funding application but not requesting cord blood products for research. In the event that the project co-investigator needs to have the cord blood samples shipped directly to them, the co-investigator will need to submit a separate project application as the PI.

Project Summary

Provide a detailed summary of the research project, indicating the rationale, the hypothesis or research question, the significance of the study (i.e., overall anticipated public and/or scientific benefit), the primary outcomes/goals of the study, and a short explanation of how the research aligns with the goals of the Cord Blood for Research Program (maximum of 300 words).

Lay Project Summary for the Public

Provide a summary of the research project in lay terms (maximum of 150 words). If the project is approved, this lay summary will be published on Canadian Blood Services’ website to inform potential donors about the research that is supported by the Cord Blood for Research Program. The project will not be approved until the lay summary is determined to be at an appropriate level for the public.

Project Alignment to the Goals of the Cord Blood for Research Program

Indicate how the expected outcome for your research project aligns with Program goals.

Funding

Indicate the source of funding (if any) for the research project.

Cord Blood Products Requested for the Project

Provide information on the total number requested and the rationale for this number, the anticipated delivery schedule including estimated start date, and any special requirements.

Billing Information

Cord blood for research products are provided on a cost recovery basis. Please provide information for invoicing purposes.

Contact person for Shipping Purposes

The person who would be contacted regarding shipment and receipt of cord blood for research products and the shipping address. 

Section 2: Research Ethics Information

Institutional Research Ethics Review

Provide institutional research ethics review and approval information. A copy of the Research Ethics Board application and approval letter must be included as part of the submission to the Program.

Stem Cell Oversight Committee Review

Provide information regarding Stem Cell Oversight Committee approval, or why Stem Cell Oversight Committee approval is not required.

Section 3: Principal Investigator Certification

Understanding of Research Consent

The Principal Investigator must certify that they have read and understood the “Permission to Collect with Opt In for Research” and the “Information for Cord Blood Donation for Biomedical Research” pamphlet (PDF). Please contact us at researchcordblood@blood.ca for information on how consent is obtained.

Project Application

The Principal Investigator must certify that all of the information provided is accurate and complete and agree to accept responsibility for the scientific conduct of the proposed research study.
