Sample / Requisition Acceptance Criteria

The Diagnostic Services Laboratory reserves the right to refuse samples that do not meet minimum standards.

Sample / Requisition Acceptance Criteria (Generic)

Minimum Sample Labelling Requirements

  • Patient last and first name (key identifier)
  • PHN - Personal Health Number (key identifier) or other unique identification
  • Date of sample collection

Sample / Requisition Labelling Rejections

  • Unlabelled
  • Sample labelled with more than one name
  • Requisition and sample labelling mismatch or missing key identifiers (Name / PHN or other unique identification)
  • Missing date of collection / phlebotomist initials

Requisitions

Request for Perinatal Testing

  • Ensure all information is clearly printed or if an information label is used, that it provides all required information. Sample and requisition must match.
  • Indicate the hospital for delivery.
  • Document date of injection if the patient has received RhIG this pregnancy.
  • In order to facilitate test result reporting, it is important that the physician’s information is complete – especially the name, clinic name and fax number.
  • Phlebotomist signature/identification and collection date must be included.

Request for Crossmatch/Antibody Investigation

  • Ensure all information is clearly printed or if an information label is used, that it provides all required information. Sample and requisition must match.
  • Document all required information; including diagnosis, previous transfusion information and medications.
  • Phlebotomist signature/identification and collection date must be included.
